What to Expect
Breast reconstruction surgery is performed in various stages. In certain phases (like the flap creation, inserting an expander, or placing implants), general anesthesia may be used. Some patients will need to stay overnight, but this is based on the treatment phase. Once the last step has been completed in the breast reconstruction process, you will wear a supportive bra and you will be prescribed medication by Dr. One to help control discomfort, bleeding, and swelling. Over time, the reconstructed breast(s) should appear more normal so you can feel more confident. Regular monitoring with mammograms and breast exams are essential and highly recommended for your ongoing health.
Surgical Technique
There's many surgical techniques available today to reconstruct the breast. There are also many factors that Dr. One considers when determining the technique that will produce your best results. Some of these factors include cancer diagnosis, when the reconstruction is performed, whether you will use implants or natural tissue, and if nipple-sparing or nipple reconstruction is a part of the process. In order to rebuild the breast, there must be enough tissue to adequately cover the breast mound. Flap methods accomplish this by using your own fat, muscle, and skin to create, cover, and build the mound. The most frequent flap techniques are:
- TRAM Flap
This method takes skin, fat, and muscle from your abdominal area to create the breast. The tissue used to build the new breast may be detached from the abdomen or the tissue may stay attached to the donor location, maintaining the original blood supply. - DIEP Flap
Like a TRAM Flap, a DIEP Flap removes fat, skin, and blood vessels from the lower stomach, but it doesn't use the muscle. The DIEP (deep inferior epigastric perforator) flap moves fat and skin from the belly to build the new breast then uses microsurgery to attach the transferred blood vessels to the chest. - Latissimus Dorsi Flap
This technique uses muscle, fat, and skin from your back. During this procedure, tissue is taken from the back to the mastectomy site through a surgically created path so it stays attached to the donor location, which keeps the original blood supply intact. - PAP Flap
Another flap is the profunda artery perforator (PAP). This technique uses fat, muscle, and skin tissue from your inner thigh to reconstruct your breast(s). - LTP Flap
While the TRAM flap is one of the more common methods used during reconstruction, sometimes there isn't enough muscle, fat, and skin that can be taken from the abdominal area. In these situations, the LTP flap may be performed, which uses tissue from the lateral thigh. - Composite Flap
Also known as a composite stacked flap, this method takes flaps from several sections of your body to reconstruct the breast.
